This guide, as a document
Everything on this page, laid out to print. Use your browser's print or save-as-PDF. No email, no sign-up.
- Every line of business, with what to watch for on each.
- What we checked, and what we could not verify.
- Kept current: it is the same catalogue we work from.
It opens in a new tab. Then use your browser's print or save-as-PDF to keep a copy with your file.
The checklist is the work
This page is the map: the channel, the order it happens in, the traps, what the payer publishes about time, and what we could not verify. It is not the checklist.
We do not publish the itemised list of documents and conditions for each line of business. You will not find it on this page, and you will not find it by reading. We hand it over instead: resolved for the line you are actually filing, and written down against your own file. It is the part that takes the work, and it is the part that goes wrong when nobody has done it.
The first step is $250: we read your documents against what Amerigroup asks for and tell you in writing what is missing, before anything is filed. It is credited in full against any engagement.

Tell us who you are and we will send you the whole file: the conditions that must be true before you file, and every document the payer asks for, line by line. It costs nothing and commits you to nothing.
Where you file
- How you apply
- GAMMIS (Medicaid, via the DCH Centralized CVO) + Amerigroup digital provider enrollment in Availity
- The path
- Medicaid: enrol in Georgia Medicaid via GAMMIS → DCH CVO credentialing → Amerigroup network enrolment through Availity.
- What access you need
- Amerigroup Provider Services (Georgia) 1-800-454-3730 · Georgia provider representative 1-800-249-0442 · Availity Client Services 1-800-282-4548
- Who verifies
- Georgia DCH Centralized CVO — Amerigroup does not credential Georgia Medicaid internally
- Worth knowing
- Amerigroup's own Georgia provider page states that all individual practitioners and facilities seeking to enrol with a Georgia CMO are credentialed and recredentialed through the DCH Centralized CVO, and that the application is submitted through the GAMMIS web portal. The CVO's decision is what Amerigroup acts on.

Lines of business
Each line is its own application, with its own checklist, its own effective date and its own contract. Filing one does not file the others. What follows is what this payer asks for, line by line.
Georgia Medicaid (Georgia Families / 360)
The DCH CVO decides; Amerigroup contracts. Amerigroup is one of the three CMOs DCH contracts with today, but its contract is extended only to 30 June 2027 while the procurement is unresolved — see the transition gate on this filing.
What to watch for
The things that quietly stop a file, whichever line of business you are filing. We check each one before we file.
- Must be trueTRANSITION GATE — Amerigroup is an incumbent whose DCH contract runs only to 30 June 2027. DCH has not issued a Notice of Award and has published no implementation date. Confirm the current CMO list on DCH's own page and decide whether this contract is worth the risk. Our own bar, not a published payer rejection rule. The facts are DCH's own: the CMO procurement is in the protest phase pending the Notice of Award, and DCH extends the current CMO contracts through 30 June 2027 (SFY2027). Amerigroup was NOT among the apparent successful health plans, so it is the one current CMO whose contract ends on a published date.
What the payer publishes about time
These are the payer's own published figures. They are not our commitment, and they are not a promise about when you can bill.
- Completeness review
- The CVO completeness review gates the clock — the 45 days runs from a COMPLETE application.
- Decision
- DCH Centralized CVO: decision within 45 calendar days of a complete application. Amerigroup's contracting step runs on its own clock after that decision.
- Committee
- DCH CVO Credentialing Committee (Georgia Families and Georgia Families 360°)
- Contract
- Amerigroup issues its provider agreement after the CVO decision
- What the numbers mean
- One CVO decision covers every Georgia CMO. Amerigroup's own contracting is a separate, shorter step. The CVO's own missing-information response window is not published — chase it, do not assume one.
Deadlines that start a clock
These run against a filing. Miss one and the filing stops, or restarts.
Amerigroup's DCH contract ends — 30 June 2027 at the latest
Recredentialing
- Cycle
- 36 months
- What it hangs on
- NCQA / CVO cycle
- Worth knowing
- The DCH CVO Credentialing Committee handles recredentialing for Georgia Medicaid providers. DCH notifies providers in advance; the exact notice period for this file is not separately published.
The effective date
Approval is not the date. These are the markers that make a date real.
- How it is established
- The CVO decision, then Amerigroup's provider agreement, then the roster load. Amerigroup's roster load is the point at which members can actually be billed.
- What proves it
- Amerigroup provider agreement, then roster confirmation on the Georgia Families roster
- Contract date
- This is not the same as the contract effective date. Track both.
- Worth knowing
- Do not record an effective date from the CVO decision alone. The CVO decision is the credentialing; Amerigroup's contract and roster load are separate, and each has its own date.
What we checked, and what we did not
Read this before relying on anything else on this page.
GAMMIS and DCH CVO route, Availity digital enrolment, the provider services and representative numbers, the DCH CVO email address, DCH's own current list of contracted CMOs, the 30 June 2027 contract extension and the 1 July 2026 revalidation suspension confirmed against DCH, Amerigroup and GAMMIS primary pages on 2026-11-04. The contract end date is recorded as DCH's published extension of the incumbent contracts, not as a clause read off an Amerigroup provider agreement.

Information and forms
- Georgia DBHDD — Become a Provider (Community Behavioral Health)Georgia Medicaid behavioural health has its OWN state enrolment, separate from GAMMIS and from the health-plan contract: a New Provider Enrollment Forum, a Letter of Intent during an open enrollment period, an application, and a site visit if one is required.
