Prepared. We prepare the application and the documents for your review and signature. You submit it and manage the follow-up.
Submitted. We prepare it, submit it, and follow it to the payer's documented decision.
Effective. We prepare it, submit it, and chase it until the payer gives you a date in writing that you can bill against.
Every price in this table is one rate applied to the work that scope takes. The published number is the number you pay — we do not add fees later, and the price goes in writing before any work starts.
Coverage add-ons
Medicaid and behavioural work are a different job, priced on the same terms at the same three depths.

Payer approval or an effective date — the payer decides both
CAQH: attestation readiness and provider review support. Marlien completes supported fields, uploads client-provided documents, identifies missing information, provides a review summary, guides the provider to the required attestation step, and records the date after completion. The provider remains responsible for confirming accuracy and completing any required certification or attestation.

How the commitment works
Start date: the later of our written confirmation that required documents are complete and the first date a payer allows filing.
Due date: 10 business days after the start date.
If we miss it: the engagement receives three additional months of credentialing maintenance at no charge. This applies once per engagement and is the sole remedy for a missed submission deadline.
What payers decide
Approval, network participation, processing time, and effective dates belong to the payer. Marlien does not guarantee them. The remedy covers Marlien's submission timing only.

Credentialing questions
Payers set their own processing times, so Marlien cannot promise one. We submit within the commitment and follow up until the payer records a decision.
No. Payers decide approval, participation, timing, effective dates, and payment. Marlien commits to the work on its side, including the application submission commitment on this site.
Submission means the application reached the payer, and we keep proof of it. Approval is the payer's decision.
We prepare the applications and coordinate signatures. The provider or authorized official signs. Marlien never signs on anyone's behalf.
Some payers limit new providers. If a payer is not accepting applications, an application cannot be filed until the payer reopens. That decision belongs to the payer.
Your written scope lists the documents for your payers. Send them through the secure intake process after the engagement begins, not through website forms.
Choose a service and request your agreement. We confirm scope and price in writing, then send secure intake instructions once the engagement begins.
