How to Get a New Practice Credentialed in Texas
Texas Medicaid applications run through PEMS, and the TMHP manual says providers must be enrolled in Texas Medicaid before enrollment can be approved for Medicaid managed care. A launch sequence covers entity and provider readiness, CAQH, Medicare, Texas Medicaid, managed care, and commercial payers. Payer paths differ, and state approval does not control commercial-payer participation.

The sequence
- ReadinessEntity, tax, location, license, NPI, taxonomy.
- Provider files and CAQHComplete and current.
- Medicare and Texas MedicaidMedicare, then the right PEMS application type.
- Managed careAfter state enrollment is in place.
- Commercial payersThrough each verified payer path.
- Confirm setupEffective dates, directories, billing.
1. Entity, tax, ownership, location, license, NPI and taxonomy
Settle the legal entity, tax identity, ownership, service locations, licenses, and NPI and taxonomy details first. Mismatches here are a common reason applications are returned.
2. Provider files and CAQH readiness
Collect current provider records and bring each CAQH profile to complete, attested status with the intended payers authorized.
3. Medicare, and the Texas Medicaid application type in PEMS
Medicare is enrolled through PECOS when it applies. Texas Medicaid is applied for in PEMS, and choosing the correct application type matters. Submitting in PEMS is not enrollment: the state decides, and an enrollment is complete when the request reaches Closed-Enrolled status.
4. Medicaid managed care after the state step
Managed care enrollment follows state enrollment, is a separate step with each health plan, and is not automatic.
5. Commercial payers
Request commercial participation only through payer paths that have been verified as current. Payer participation and network availability are the payer's decisions.
6. Track milestones and confirm setup
Track submitted, acknowledged, correction requested, approved, effective, and activated as separate milestones. After the effective date, confirm directory listings and billing readiness.
Common questions
No. The state reviews the application and issues the decision and dates.
Do not plan on any fixed window. The manual says up to 60 days after all information is received, with extensions possible, and TMHP has published longer current times. See the Texas Medicaid timeline.
No. State enrollment is a prerequisite, and each health plan decides participation.
Next step
You are not charged when you build a plan. Payers and state agencies control review, participation, approval, effective dates, and timing. Marlien does not negotiate payer contracts, reimbursement rates, fee schedules, or participation terms.
